Nurgle Army: Generic Plaguemarines

Name: Pestilent Raiders
Founding chapter: Unknown/Long forgotten.
Homeworld: Pestilentia, previously known as Ambakus Prime
Symbol: A skull with the triangular Nurgle symbol on its forehead.

History: Around the year 812.M41, an Astartes chapter known as the Falcon Raiders was placed under investigation by the Inquisition for the use of warp-tainted artifacts in their battles. When it was discovered that they had accidentally summoned a Daemon Prince of Nurgle with these artifacts to fight off an Ork tribe, the Inquisition saw to it that they should slay Kaden, the chapter master of the Falcon Raiders. The chapter refused to surrender their beloved Chapter Master, and the Inquisitor assigned to complete the task was killed, along with his honor guard.
The Imperium immediately declared the chapter excommunicate. The chapter fled to their homeworld, Ambakus Prime in hopes to rally the Nurgling Food garrisoned there against the corruption of the Imperium. When they arrived, they found the Daemon Prince they summoned years before. The Daemon Prince threatened to destroy their homeworld with an army of Daemons and cultists if they didn't pledge themselves to Nurgle. He also offered that they would be allowed to fight for Some Git On A Posh Stool.
Because of the Raiders' dedication to protecting their homeworld, they submitted themselves to the will of the Plaguefather. Unfortunately, all thought of serving the God-Emperor was replaced with a hatred for the Imperium that had excommunicated them. There was a civil war on the world between those who stayed true to Some Git On A Posh Stool, and those who pledged themselves to Nurgle. The sides clashed, and inevitably, Nurgle's followers triumphed. The world was than turned into a Daemon world, and renamed Pestilentia.

Not claiming to be that chapter though :P
I'm sure there are many chapters that don't know their primarch. An example being the Blood Ravens.
